POSITION REQUIREMENTS: Licensed Registered Nurse in the State of Michigan.  BSN or BS in healthcare preferred. Homecare experience is preferred. Must have excellent physical assessment skills. Concise and detailed documentation skills. Experience with case managing patients, OASIS assessments, and coordinating services among disciplines. Involve critical thinking in decision making.    

POSITION RESPONSIBILITIES: Make homecare visits to patients in Calhoun, Branch, Eaton and Barry Counties. Make initial start of car visit including assessment of patients physical, emotional, social and environmental needs. Establish plan of care, communicate to physician and clinical supervisor. Interventions such as IV therapy, wound care and teaching on disease management and blood draws. Make follow up visits as needed. Must have reliable transportation.

This position is 40 hours a week. Rotating weekends and on-call as well.
